Additional performance review reporting options per employee

In Performance management, under Reports → Employees, the most recent evaluation is shown per employee.

Desired solution:
- A toggle to show either all evaluations or only the latest one
- The same choice between all evaluations and only the latest one when exporting, for both the CSV and the Excel export
- Two additional columns in the overview: the final score of the evaluation and which form was completed

And mabye a filter on "submitted at" so the overview can be limited to evaluations submitted within a specific period

Impact:
- Development over time is hard to follow, since only a single evaluation is visible per employee
- Extra manual work to look up historical evaluations elsewhere or to assemble them for analysis outside the platform
- Without the form name, it is unclear which evaluation cycle or template a score belongs to, especially when multiple forms are in use
